Learn how to create, preview, launch, monitor surveys, and more with this walkthrough of the Survey feature

Surveys let you collect structured input from individuals or groups directly within the Tasks page. Use them for feedback, check-ins, board evaluations, or to request missing information—all in one streamlined workflow.

Surveys are under Tasks. Once viewing your tasks select Request > Surveys from the dropdown. This will take you to the survey builder.

💡 Only secretaries and admins can create surveys

Create a Survey

  1. Start from the Tasks page

    • Select Request > Survey from the top-right corner.

    • A new draft survey will be created automatically.

  2. Build your survey

    • Use the left-hand panel to drag question types into the survey.

    • You can edit the survey in many ways, including title, description, and question text, as well as add pages and question descriptions.

    • The survey builder autosaves every time you make a change (e.g. adding questions or editing text).

  3. Tabs in the builder

    • Create: Add or edit questions.

    • Preview: See the survey from an assignee's point of view.

    • Launch and analyze: Assign recipients, set a due date, add context, and launch.

Preview Your Survey

To preview the experience as an assignee:

  1. Go to the Preview tab in the builder.

  2. You'll see how the survey will appear once sent, with your organization’s branding applied.

  3. The Complete button is hidden in preview to avoid confusion.

💡 Any changes to your organization’s branding (like color themes) will automatically apply to surveys in Preview.

Launch a Survey

  1. Open the Launch and analyze tab.

  2. Fill in the following:

    • Assignees: Choose one or more recipients.

    • Due date (optional).

    • Additional information: Add context for assignees.

    • Attachments (if needed).

  3. Click Launch

    • The Launch button is only enabled once you've added at least one question and one assignee.

    • Assignees receive a notification and the task appears in their dashboard and task list.

💡 Good practice: Before launching your survey, ensure you check everything. Once the survey is launched, you are unable to make any changes.

Respond to a Survey

As an assignee, you'll:

  1. See the survey in your Dashboard and Tasks list.

  2. Click Take survey to begin.

  3. Fill out the questions, and click Complete when you're done.

Once you complete the survey:

  • You'll see a Thank you screen.

  • If you try to open it again, you'll see an Already completed message instead.

If you're the final assignee to complete the survey:

  • The survey status will update to Completed.

  • The survey creator will receive a notification on each assignee’s completion as well as completion of the survey.

💡 Got a long survey respond to? Why not spread it out over a number of sessions with answers automatically saved. However, once answers have been submitted, participants cannot come back and amend them.

View and Track Survey Progress

As the survey creator or admin, you can:

  • Open the survey at any time from the Tasks list.

  • Navigate between the Preview, and Launch and analyze tabs to make changes or monitor progress.

  • Return to the Tasks list at any time using the Back button in the top bar.

  • You may Close the survey from the Launch and analyze screen. Any participants who have yet to submit their responses will no longer be able to answer.

Exporting, Downloading and duplicating a survey

You might find it useful to download a survey or even copy a duplicate. Both these options are found on the Launch and analyze screen once the survey has been created.

This takes the survey in its form as you created it and either:

  • downloads the survey as a pdf onto your system

  • duplicates it into an newly editable survey which opens immediately

Exporting results

There are two was you can export results, split individually or aggregated:

  • By using the Export results button in Launch and analyze screen, you will receive an XLSX file of all responses aggregated into a report.

  • Alternatively, use the Export survey icon next to each respondent to download a copy of specific users responses as a pdf.



Anonymous surveys

Create surveys that protect the identity of respondents. Perfect for sensitive topics like board evaluations or feedback where anonymity is important.

What happens when a survey is anonymous?

When Anonymous responses is checked during setup, the following applies:

  • Individual responses cannot be downloaded
    You’ll only be able to export aggregated results—and only once the survey is marked as Completed.

  • Respondents are shown as "Anonymous"
    Names are removed from the results file and replaced with “Anonymous.” The order of responses is randomized.

  • No hover-to-download option on individual responses.

  • "Anonymous survey" pill
    This replaces the usual “Survey” label in:

    • The Launch and analyze tab

    • The Details tab for assignees

    • Dashboard cards for both creators and assignees

    • The Survey tab (top-right, next to autosave icon)

  • Duplicating the survey carries over the anonymous setting automatically.

💡 You cannot change the anonymous setting after the survey has been launched; however, you can duplicate, alter the setting, and relaunch the survey.

How to make a survey anonymous

  1. Go to the Launch and analyze tab while your survey is still in draft.

  2. If your organization has this feature enabled, you’ll see a checkbox for Anonymous responses.

  3. Tick the box to anonymize the survey.

⚠️ Once a survey is in progress or completed, this checkbox is disabled.

Closing an anonymous survey

To help preserve anonymity:

  • You can only close an anonymous survey once more than one assignee has responded.

  • If fewer than two responses are received, the Close survey option is disabled.

FAQs

Can I edit a survey after launching?

No, once launched, surveys are locked for editing.

Can I assign surveys to groups?

Yes, you can assign surveys to individual users or groups. Only active users can be selected.

What happens if no assignees are added?

You won’t be able to launch the survey. A banner will prompt you to add at least one recipient.
